You need to add two (2) sections to your web sites HTML to correctly link and popup the web-form:

• The java sccript function popup - add to the top of the page.
• The correctly formatted HREF line to link to the form and cuase it to popup.

or includes spaces in the wrong place then your web-form will not appear in a popup window,
